2012 Contribution Statements

  2. If you have an account,  click sign in and you will be directed to your contribution page. 
  3. Be sure to select 2012 as your contribution year, click "view" and download your statement.
  4. If you do not have an account, click "sign up" and you'll be able to get your information within 24 hours of setting up your account.
